It kinda looks like your door is too low instead of your fender being too high. Could just be the camera angle but looks like you have a similar issue at the rear of the cab to the bed. Body panel alignment is such a pain.

Is it an original fender ? or an aftermarket replacement?
The replacement fenders sometime need to have holes filed over sized.
How are the cab mounts ? weak or rusty cab mounts can make it difficult to line up fenders.

Let's see if I got 'em all. Reference this diagram: http://www.fordification.com/tech/image ... fender.jpg

One on bottom bolts to the underside front of the cab. Top left on the diagram.
5 or 6 across the top bolts to the inner fender. Center left on the diagram.
And the infamous "inside the door jamb" one bolts near the top from inside the access hole in the door jamb. Careful with that one! If it breaks you've got a world of heartache in store. Top center on the diagram.
There may be another one or two inside the fender bolting to the cab but I don't think so.

The outer fender bolts on top of the inner fender so you may have to loosen those bolts, along with the other fender bolts to get it all to drop down enough. Hope this helps.
